A good Patient Note is essential for passing the STEP 2 CS exam. It determines whether you pass the ICE (Integrated Clinical Encounter) component of the exam (others being CIS and SEP). Many good students/doctors fail in the ICE component due to lack of adequate data gathering skills and inability to type concise yet effective patient notes.
Data Gathering is a multiple step process. In the short span of 15 min allotted for the clinical encounter with the standardized patient, the doctor is expected to ask a series of questions - not just any random question based on popular mnemonics but a specific, focused set of questions (focused history) pertinent to the chief complaint (

). The line of questioning may have to vary along the course of history taking as the patient reveals information (Step 5 of our online video tutorial demo). The answers to these questions are to be mentally noted or recorded on your blue sheet. Your job does not end here! The Physician who grades your ICE has no clue what your asked your patient unless you transfer the information onto the patient notes that you have typed under time pressure in 10 min.
